ستاتين
الستاتينات statins (أومثبطات HMG-CoA reductase ) يشكلوا رتبة من الادوية الناسيرة لشحميات الدمs تستخدم لخفض مستويات الكولسترول في الأشخاص الذين يعانون من، أوالمهددون ب، سقم قلبي وعائي. فهم يخفـِّضوا الكولسترول بتثبيط الإنزيم HMG-CoA reductase, الذي is the يعتبر خطوة محددة لمعدل مسار ميفالونات المساهم في تصنيع الكولسترول. ان تثبيط عمل هذا الانزيم في الكبد تستثير مستقبلات البروتين الشحمي الخفيض الكثافةمما ينتج عنها زيادة في عملية تصفية بروتين شحمي منخفض الكثافة من الدم مما ينتج عنه خفض مستوى الكولسترول بالدم.يمكن ملاحظة هذه النتيجة بعد اسبوع واحد من استعمال العقار ويكون شدة تأثيره بعد اربعةالى ستة .
تاريخ
أكيرا إندووماساوكورودا من طوكيو, اليابان commenced research into inhibitors of HMG-CoA reductase in 1971 (Endo 1992). This team reasoned that certain microorganisms may produce inhibitors of the enzyme to defend themselves against other organisms, as mevalonate is a precursor of many substances required by organisms for the maintenance of their cell wall (ergosterol) or cytoskeleton (isoprenoids).
آلية العمل
يعمل ستاتين على تثبيط انزيم HMG-CoA reductase, هذا الانزيم الذي يتحكم في المستوى الاولى في تصنيع مادة الاستيرول(كولسترول)في الكبد, . ولما كان ستاتين مشابه ل HMG-CoA on a في المستوى الجزيئي tلذا فهويحل محله في الانزيم ويقلل معدله الذي يمكنه من تصنيع الميفالونات, الجزيئ التالي الذيقد يكون تلقائيا مادة الكولستيرول, وعدد اخر من المركبات.
وفي داخل الكبد, تشعر بعض انزيمات البروتياز بنقص مستوى الكوليسترول . فينتج عن هذا تشقق نوع من البروتين اسمه "membrane-bound sterol regulatory element binding protein", والذي يستجيب بعد ذلك بالانتنطق الى للذرة للعمل على زيادةانتاج انواع مختلفة من البروتينات والانزيمات , بما فيها مستقبلات البروتين خفيض الكثافة. هذه المستقبلات تتخذ مكانها على الجدار الخلوي للخلية الكبدية , حيث تتحد هناك لتكون بروتين شحمي خفيض الكثافةLDL وجزيئات بروتين شحمي وضيع الكثافةVLDL (كلاهما يحتوي على الكوليسترول على شكل غير مرغوب فيه). LDL وVLDL يدخلان الكبد ويتم هضمهما.
دواعي الاستعمال وطريقة الاستخدام
تعتبرالستاتينات اكثر العوامل الخافضة للكوليسترول المتاحة, , فهي تقوم بخفض LDL كوليسترول خفيض الكثافةوالذي يطلق عليه ( "الكوليسترول السئ") بنسبة 1.8 mmol/l. مما يعني انخفاض عدد النوبات القلبية بنسبة 60% (نوبة القلبية, الموت القلبي المفاجئ), ويقلل نسب حدوث السكتة القلبيةبنسبة 17%. وهي ذات تأثير اقل من تأثيرال مكونات الاليافsاونياسين في تقليل ثلاثي الجليسريدs والذي يحمل مستوى HDL-البروتين الشحمي المرتفع الكثافة ("الكوليسترول الجيد"). اذا فالدليل العملي الارشادي هناات المريض حاول عمل حمية مخفضة للكوليسترولقبل استخدامه للستاتين ; اوعوامل فارماكولوجية اخرى قد يوصى بها للسقمى الذين فشلوا في خفض مستوى الدهون عن طريق الحمية والنظام الغذائي السليم .

LDL-lowering potency varies between agents. Cerivastatin is the most potent, (withdrawn from the market in August, 2001 due to risk of serious rhabdomyolysis) followed by (in order of decreasing potency), rosuvastatin, atorvastatin, simvastatin, lovastatin, pravastatin, and fluvastatin. The relative potency of pitavastatin has not yet been fully established.[]
Some types of statins are naturally occurring, and can be found in such foods as oyster mushrooms and red yeast rice. Randomized controlled trials have found these foodstuffs to reduce circulating cholesterol, but the quality of the trials has been judged to be low. Due to patent expiration, most of the block-buster branded statins have been generic since 2012, including atorvastatin, the largest-selling branded drug.[]

تفاعلات الأدوية
Combining any statin with a fibrate, another category of lipid-lowering drugs, increases the risks for rhabdomyolysis to almost 6.0 per 10,000 person-years. Most physicians have now abandoned routine monitoring of liver enzymes and creatine kinase, although they still consider this prudent in those on high-dose statins or in those on statin/fibrate combinations, and mandatory in the case of muscle cramps or of deterioration in renal function.
Consumption of grapefruit or grapefruit juice inhibits the metabolism of statins—furanocoumarins in grapefruit juice inhibit the cytochrome P450 enzyme CYP3A4, which is involved in the metabolism of most statins (however it is a major inhibitor of only lovastatin, simvastatin and to a lesser degree atorvastatin) and some other medications (it had been thought that flavonoids were responsible). This increases the levels of the statin, increasing the risk of dose-related adverse effects (including myopathy/rhabdomyolysis). Consequently, consumption of grapefruit juice is not recommended in patients undergoing therapy with most statins. An alternative, somewhat risky, approach is that some users take grapefruit juice to enhance the effect of lower (hence cheaper) doses of statins. This is not recommended as a result of the increased risk and potential for statin toxicity.

Cholesterol is not water-soluble, and is therefore carried in the blood in the form of lipoproteins, the type being determined by the apoprotein, a protein coating that acts as an emulsifier. The relative balance between these lipoproteins is determined by various factors, including genetics, diet, and insulin resistance. Low density lipoprotein (LDL) and very low density lipoprotein (VLDL) carry cholesterol toward tissues, and elevated levels of these lipoproteins are associated with atheroma formation (fat-containing deposits in the arterial wall) and cardiovascular disease. Conversely, High density lipoprotein (HDL), carries cholesterol back to the liver and is associated with protection against cardiovascular disease.
Statins act by competitively inhibiting HMG-CoA reductase, the first committed enzyme of the HMG-CoA reductase pathway. By reducing intracellular cholesterol levels, they cause liver cells to make more LDL receptors, leading to increased clearance of low-density lipoprotein from the bloodstream.
Direct evidence of the action of statin-based cholesterol lowering on atherosclerosis was presented in the ASTEROID trial, which demonstrated regression of atheroma employing intravascular ultrasound.

Statins exhibit action beyond lipid-lowering activity in the prevention of atherosclerosis. Researchers hypothesize that statins prevent cardiovascular disease via four proposed mechanisms (all subjects of a large body of biomedical research):
- Improving endothelial function
- Modulate inflammatory responses
- Maintain plaque stability
- Prevent thrombus formation
In 2008 the Jupiter study was halted early due to a large benefit seen in the statin treated group. This randomized placebo-controlled international study involved about 18,000 individuals with normal cholesterol and no history of heart disease, but with elevated C-reactive protein levels. Investigators found reductions in stroke, heart attack, and need for bypass surgery of about 50% in the statin treated group. There was also a 20% reduction in mortality.
خلاف
Some scientists take a skeptical view of the need for many people to require statin treatment. Given the wide indications for which statins are prescribed, and the declining benefit in groups at lower baseline risk of cardiovascular events, the evidence base for expanded statin use has been questioned by some researchers. A much smaller minority, exemplified by The International Network of Cholesterol Skeptics, question the "lipid hypothesis" itself and argue that elevated cholesterol has not been adequately linked to heart disease. These groups claim that statins are not as beneficial or safe as suggested.
